These 22 Mishaps While Filming Ended in Injury and Worse
Walt Disney Studios Motion Pictures
The recent, tragic death of Halyna Hutchins on the set of Rust reminds us of how often things can go wrong while shooting a film or a TV show. Not everything is propitious about filming scenes on a set. Things can go awry. And we're not just talking about the stuff of gossip columns. We're talking about perilous mishaps on set. Did you know that George Clooney was injured on the sets of Syriana? Or that Bruce Willis lost part of his hearing on the set of Die Hard. Here's a list of 20 such mishaps on set that prove filming isn't all that free from strife. (TW: gun violence, death, near death by accidental hanging, auto accident, and other violent accidents)

Warner Bros. Pictures

Brad Pitt is known to give more than a 100 percent towards his on-screen performances. His popular films include Thelma & Louis, 12 Monkeys, and Fight Club. Charlize Theron – Aeon Flux
Paramount Pictures Studios
Charlize Theron – Aeon Flux
Did you know Charlize Theron suffered an (almost) deadly fate while filming the 2005 film Aeon Flux? While performing a handspring back flip in platform shoes—a stunt Theron was unprepared for—she fell on her neck and slipped a disc. The disc was scarily close to damaging her spinal cord and paralyzing her permanently. Luckily, that did not happen, and she recovered in due time.
Halle Berry – Gothika

Bruce Willlis – Die Hard
20th Century Studios
Bruce Willlis – Die Hard
Are you a Die Hard fan? Did you know that while filming a scene, Bruce Willis ended up firing a round from a gun in a closed environment? What happened? Despite making several other films, Willis has exclaimed how his hearing loss has affected his overall life.
Uma Thurman – Kill Bill Vol 2
Miramax
Uma Thurman – Kill Bill Vol 2
Uma Thurman was behind the wheel when her rehearsal car stunt for Kill Bill Vol 2 took a terrible turn, quite literally! Although Quentin Tarantino test drove the car in one direction, he asked Thurman to drive from the opposite direction, unaware of a dangerous twist on the road. To everyone’s horror, the car crashed. Thurman suffered severe damage to her knees and neck. For Tarantino, letting Thurman drive was one of his "most horrendous mistakes."
Gianni Russo – The Godfather
Paramount+
Gianni Russo – The Godfather
Even the The Godfather wasn’t filmed without some mishap! In one particular fight scene, Corleone (James Cann) seeks out Carlo Rizzi (Gianni Russo), and beats him to make sure he doesn't abuse Sonny's sister ever again. All the thrashing resulted in Russo cracking two of his ribs and screaming out in actual pain. While it was mostly an accident, there were rumors that Cann and Russo did not get along that well.
Tom Cruise – Mission: Impossible 6
Paramount Pictures Studios
Tom Cruise – Mission: Impossible 6
Did you know that Tom Cruise executes quite a few of the action stunts himself? In a scene from Mission: Impossible - Fallout, Cruise was meant to jump from one building to the next. The "easy" stunt, as the star explains it, was supposed to end with him slamming into the side of a building. But he landed a bit differently and broke his ankle. Nevertheless he climbed back up and finished the scene.
Johnny Depp – The Lone Ranger
NBCUniversal Television Distribution
Johnny Depp – The Lone Ranger
Johnny Depp is a phenomenal actor who can take on just about any role Hollywood throws his way. But things went a little awry while filming The Lone Ranger. Although he escaped without any serious injuries, he did suffer a nearly fatal bruise on his stomach that was in the shape of a hoof print.
Leonardo DiCaprio – Django Unchained
The Weinstein Company | Sony Pictures Releasing

Reportedly, DiCaprio received a standing ovation from everyone in the room after the take.
Drew Barrymore – Santa Clarita Diet
Netflix
Drew Barrymore – Santa Clarita Diet
When you jump onto someone's back, you either get a free piggyback ride, slide back down, or something worse happens. In Drew Barrymore's case, it was option number three. While filming a scene for Santa Clarita Diet, the Charlie's Angels star jumped on a man's back, accidentally slipped, and hit her head on a pavement. While she made a full recovery, she was hospitalized for two days due to a concussion.
Robert Downey Jr. – Iron Man 3
Walt Disney Studios Motion Pictures
Robert Downey Jr. – Iron Man 3
Being Iron Man isn't easy, not even in real life. While filming a stunt for the movie's climax scene, the A-list actor was supposed to jump from one platform to another in a drop as high as 12 feet. Unfortunately for him, he landed a lot harder than he should have and broke his ankle. The incident halted production for about six weeks before it was resumed. We thank Odin for that.
Jim Carrey – Yes Man
Warner Bros. Pictures
Jim Carrey – Yes Man
Yes Man is about Carl Allen (Jim Carrey), who says "yes" to just about anything after a positivity seminar. Things took a turn when during a scene, the Bruce Almighty actor was meant to do a pratfall and decided to spice things up. He hit the ground much harder than he should've and cracked three ribs. If you're wondering whether Carrey finished the scene, the answer is "yes man."
Adam Sandler – Uncut Gems
A24 | Netflix
Adam Sandler – Uncut Gems
Adam Sandler endured a few bruises while filming Uncut Gems. In a scene, Sandler was supposed to be choked by some thugs. Jackie Chan – Armour of God
It wouldn’t come as a surprise if we told you Jackie Chan has endured a significant number of injuries spanning his long acting career. While filming for Armour of God, the kung fu legend and entertainer was supposed to take a sip of a beer and jump from an overhang onto a tree. However, the branch gave away and he crash landed on a rock that fractured his skull. Life-threatening as it was, Chan survived!

Luckily, the set crew managed to free Fox from the rope just in time.
Channing Tatum – The Eagle
Focus Features | Universal Pictures
Channing Tatum – The Eagle
Channing Tatum faced quite a severe “privates” mishap on the sets of The Eagle. Unfortunately, the runner who handed Tatum his water cup forgot to mention that the water was boiling hot. What happened next is described as “burnt and shriveled” by Tatum.
Isla Fisher – Now You See Me
Lionsgate Films
Isla Fisher – Now You See Me
Isla Fisher probably did not expect an underwater shackle scene to turn into a near-death experience. While filming for Now You See Me, the actress had to submerge in a water tank for a stunt. Shockingly, one of the chains between her ankles and wrists got stuck between a slat. So at some point, she was actually panicking about drowning and asking for help! As for the crew, they thought the actress was nailing the act!
Angelina Jolie – Salt
Sony Pictures Entertainment Motion Picture Group
Angelina Jolie – Salt
Angeline Jolie hardly turns down the opportunity to pull off some larger-than-life stunts on screen. But it was the daring attitude that led to her injury on the set of Salt. An easy stunt went south after she misjudged the amount of space for its execution and hit her head against a desk. Jolie exclaimed how she initially thought she had suffered a concussion when she couldn't hear anyone, only to realize she still had her earplugs in.
Sylvester Stallone – Rocky IV
MGM/UA Entertainment Company | United International Pictures
Sylvester Stallone – Rocky IV
We've all watched Sylvester Stallone get punched rather harshly in all Rocky movies. However, in reality, there was one instance in Rocky IV where the actor was critically injured in real life. In an on-set fight scene with Dolph Lundgren, Stallone directed Lundgren to go at him hard. The actor bruised his pericardial sac and was rushed to the ICU where he stayed for four days before recovering.
